Health Form 2**ALL CAMPERS ARE REQUIRED TO HAVE A CURRENT HEALTH FORM SIGNED BY A PHYSICIAN. HEALTH FORMS MUST BE IN OUR OFFICE NO LATER THAN 48 HOURS PRIOR TO YOUR SESSION. ** A $50 Deposit is required with Registration form. Remaining balance is due no later than two weeks prior to session. Open Houses: Sundays, April 3, 17 and May 15 from 1:00 pm- 4:00 pm.
